Forms
are used to get information into the system. This is the
most customizable portion of the system. Most schools
have spent a good deal of time creating their evaluation
forms of both preceptors and students. You don't need
to abandon these forms just because you decide to go with
EMS. We will add your forms to the system.

You
can use many different evaluation forms. We can customize
the system so each rotation type has its own evaluation.
All custom forms remain the intellectual property of the
school that creates them.

Therefore,
if you add your own forms to EMS, they cannot be used
by any other school using EMS unless permission is given
by the originating school.
Forms are more than just evaluations. Here is a list of
some of the forms that have been customized for EMS:
- Patient
Intervention
- This allows students to record their patient
intervention activities.
- Site
Evaluation
- You can enter your site evaluation online
and have them available for immediate viewing
or reporting.
- Absence
Request - Students can let you know
when they are going to be absent during a rotation
and enter a makeup plan.
- Course
Evaluations - Evaluations don't have
to be limited to rotations; you can have your
students enter evaluations on the instructors
for their academic course
